Documents: Age and ID proof, income/employment proof, medical reports where required, photo, nominee details.

PLI Premium / Surrender / Loan Form

Pay premium at the counter, surrender a policy, or take a loan against the surrender value after the policy has run the prescribed period.

Documents: Policy bond/document, premium receipt book, ID proof; loan needs assignment forms.

PLI Maturity / Death Claim Form

PLI

Claim the sum assured on policy maturity, or a death claim by the nominee/legal heir.

Documents: Original policy bond, death certificate (for death claims), claimant ID, discharge voucher.
